  3. Psychoneurological dispensary certificate (Odesa, 27 Kanatna str.).
  4. Drug dispensary certificate (Odesa 26 Breusa str, korp.2).
  5. Military records book.
  6. Photos 3*4 (4-5).
  7. Seaman’s book and Ukrainian Passport.
  8. Direction from the company.
